FormWizard is a MediaWiki extension that allows users to create forms on MediaWiki wikis. The forms is then used to create new pages.

The extension currently does not have logo and it'd be nice to have one.

In this task, your work is to create a nice logo for the extension.

The design should reflect the purpose of the extension as described on its documentation page.

Some important things to note:

• Submissions must be free and must not include any non-free materials. Please mention the license under which you put the file (such as a Creative Commons Attribution 4.0). You can not re-use another author's design work, except for when it is under a CC-0 license. If you use work by another author, then you must provide a link to that work here.
• The design must be submitted as a vector .svg file (which can be resized without pixellation). The design must not include any bitmap graphics (examples for bitmap file formats: PNG, JPEG, BMP, TIFF, WEBM, etc). You are not allowed to convert a bitmap file to an SVG file.
• Upload your design on the corresponding Phabricator task linked here on GCI site. (If you do not have a Wikimedia Phabricator account yet, create one here)
• Do not forget to submit your work here also after you're thorough.
