PostgreSQL US - Develop an Integrated Form

The United States PostgreSQL Association, affectionately known as PgUS, is a IRS 501(c)(3) non-profit public charity that has the intention of supporting the growth and education of PostgreSQL, The World's Most Advanced Open Source Database, throughout the United States.

Your task will be to fork the official repository (https://github.com/pg-us/pgusweb) hosting the PostgreSQL US website and send us a link to your forked repository that integrates the currently existing Diversity Scholarship form hosted on Google Forms (https://docs.google.com/forms/d/1dgFUVXHgJ0sLAETJaAciLFetvFq5_S0-juZ3g8QyeVc/edit?ts=58f14198) into a web page of its own.

The source website uses Django as a framework; accepted submissions will need to at minimum consist of a self-sustaining set of HTML, CSS, and/or JS files. Preferably, it would be ideal if you could adapt the created form to fit within the currently existing Django template.
