Design an informative 404 error page for OpenMRS

Currently, 404 Page Not Found errors in OpenMRS lead to the default apache error page. For the OpenMRS Reference Application, we'd like to design a more informative error page.

Using a UI designer tool of your choice, design an informative error page that could be used for OpenMRS Ref App!

Submit your design as an HTML code file, a zip file if you require external files, or as an accessible link to the page. Please be creative; however, if you are using outside images, don't forget to attribute them according to Creative Commons Attribution.
