Propose a new design for the QATracker entry page

The results / entry page has gotten a bit long and messy. Here's an older mockup http://imgur.com/UCyuoZk done by Pasi that has plenty of good ideas. Take some of these and incorporate your own to propose some UI overhaul for the tracker.

I'd like to see several mockups for your ideas as images. Also feel free to consider tweaking other parts of the QATracker with the goal of making it easier to use. Include mockups of other pages you tweak.

Note, there's 2 instance counts for this. I'd like to see more than one proposal, so 2 students can have a try!
